Provide case management services that assess clients needs, develops, implements and reviews wellness plan and goals and work with community resources in meeting and achieving clients service needs.
Provide linkages between treatment and additional resources required to improve treatment outcomes and provide support for following treatment. As needed will create case plans that promote moving towards self-sufficiency.
Work as a team with other staff; support team members.
Develop a community resource list for clients.
Facilitate multiple care aspects (case coordination, information sharing, etc.)
Maintain appropriate professional and ethical standards while serving as a representative of BISAC
Job Type: Full-time
Pay: $17.00 - $18.00 per hour
Expected hours: 40 per week

Education:
- Associate (Preferred)
Experience:
- Case management: 2 years (Preferred)
License/Certification:
- CPR Certification (Preferred)
